Douglas Street Surgery is an NHS Lanarkshire GP Practice. Our priority is caring for our patients within the framework of the NHS.
We are frequently asked to produce private letters and reports which takes time which could otherwise be used for NHS work, prices for private work listed below reflect this:
- Standard 'to whom it may concern letter' (one small paragraph) £30
- Letter for school, college, university, employer or similar £60
- Report for Insurance provider which does not require consultation/examination/medical records not extensive- eg. life insurance £120
- Report for Insurance provider which does not require consultation/examination/medical records are extensive- will be charged at £150 per hour of doctors time
- Any non-specified private work will be charged at £150 per hour of doctors time
We request if you are required to make a payment, you make it via bank transfer to:

We do not perform:
- Firearms medical/certificate
- Fitness to fly letters
- HGV medical
- Occupational medicals
- Diving medicals
- Assessment of capacity for legal/property issues, eg. Power of Attorney, unless GP involvement is specifically and directly requested by solicitor
When an report about you has been requested by an external private provider (eg. a company that provides Critical Illness Cover, Income Protection, Life Insurance, Pre-employment or Retirement Occupational Medicals) we will only provide information with your consent.
There are some specific instances that information can be be lawfully share without your consent, such as information being shared with the DVLA or Police.
In instances where a report is requested by private company or government department, payment is not made to the Practice Directly.
Instead payment is made by the company to:
- to the Doctor/Individual completing the report directly (ie. their own bank account), or
- an external account as requested by the Doctor/Individual completing the report (ie. a Private Ltd company)
